Evidence-based medicine is the key to developing nursing interventions with real effect in the community. Evidence-based medicine uses results from studies to inform the best practices. This essay will present my PICOT question on my chosen clinical topic, breast cancer.

Picot Question

P- women aged 25-40 years

I- self-breast examination

C-no self-breast examination

O-reduced incidence of stage 3 and 4 breast cancer first-time diagnosis

T- 6 months

PICOT QUESTION: How will health education on self-breast examination among women aged 25 years and older compared to those not receiving health education on self-breast exam reduce the incidence of stage 3 and 4 breast cancer first-time diagnosis over six months?

Evidence-Based Solution

Breast cancer continues to cause morbidity and mortality worldwide and in the United States. Although it is a disease of both males and females, breast cancer is primarily diagnosed in females. Additionally, the incidence of breast cancer increased in women aged 20-39 years in 2018 (Ellington et al., 2022). High disease burden and mortality are attributed to late diagnosis in stages 3 and 4 of the disease. Five-year survival from breast cancer varies from place to place, and it largely depends on the public health measures and community awareness of the importance of early detection. Breast examination is a non-invasive and non-expensive intervention for early breast cancer diagnosis. According to Ngan et al. (2020), clinical breast examination led to increased early stages diagnosis and a decline of late stages diagnosis, leading to more cases of breast cancer survivors. Khiyali et al. (2017) point to a lack of knowledge on self-breast examination as the cause of the lack of self-breast examination practices among many women. Additionally, the researchers state that 40% of breast cancer cases were detected by women who reported a lump in the breast; thus, the self-breast examination could detect many instances of early breast cancer with a lump. While mammography helps detect diseases not palpable as lumps, it is not readily available and not as convenient as self-breast examination.
